What Do I Bring To My First PT Visit?

Physical therapy is not something that people experience at a particular age. Our offices work with kids, student athletes, professional athletes, adults, and seniors. As we stated, earlier, everyone deserves to be healthy. Each of our Physical Therapists is here to provide a personalized plan based on a case-by-case basis. No cookie cutter roads to wellness here.

Some items you will want to bring to your first appointment include:

  • A prescription for physical therapy from your doctor.
  • Insurance card(s)
  • Print out our patient forms, fill them out, and bring them with you (this will save some time) or we can give them to you upon arrival.
  • In case of an automobile accident or worker’s compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number.
  • Please arrive early to complete any remaining paperwork.
  • Wear comfortable clothing that is not restrictive to allow for full ranges of motion.

At your first appointment one of our expert Physical Therapists will review your paperwork, and provide a comprehensive evaluation of your condition, movement capabilities, pain points, understand your day to day, talk about your goals for therapy and your physician’s goals, and then discuss with you the appropriate course of treatment. At this point, don’t be surprised if they give you homework. These homework exercises are a way to supplement your office visits and help you towards recovering faster. Most appointments are only an hour and should be consistent with your plan of care.

Each visit is a step towards feeling like yourself again, but without the pain and limitations you started with.
